North Carolina State Lab and US Center for Disease Control (CDC) Send Out

Submission to the NC State Public Health Lab or the US CDC requires completing their respective forms, and in some cases, pre-approval from the respective agency. This includes orders placed under Serological Test to State Lab LAB6819 for tests like Zika Virus Testing. 

It is the responsibility of the ordering provider to complete the required form(s) as mandated by these agencies, and obtain the necessary approval, if required. The laboratory will not send specimens without receiving the required form completed, and if needed, evidence of pre-approval.
